HEALTH CARE PROXY

"Who will speak for you when you cannot?"

This essential document enables you to appoint a person (legally called an “Agent”) to make any healthcare decisions for you during the times you cannot communicate your wishes. The Health Care Proxy allows your Agent to provide specific instructions to the doctors regarding medical and life-sustaining treatments, based on your wishes.

A Health Care Proxy works so well because your Agent will have discussed your wishes with you in advance, and will be able to explore any options with doctors, knowing how you feel about end-of-life or comfort care.

A Health Care Proxy should not be confused with a Living Will, which consists of your written instructions regarding life-sustaining treatment.

We recommend and draft Health Care Proxies for all our clients, as they allow individuals to maintain as much control as possible over future medical treatments and decisions.
